Welcome to the School of Pharmacy Transportation Request Form. Please read and accept our room reservation policy to proceed with your request. For the disposition of your request, you will be contacted by the Business Management. The reservations are not always on a first-come first-serve basis. School official business can cause changes in priority and perhaps even cause the need to change reservations already made.
School of Pharmacy Transportation Use Policy and Instructions
- Use of these resources
Restricted to School of Pharmacy faculty and staff for official school business.
Reservations must be made at least a day in advance and no more than six weeks in advance.
- Scheduling Instructions
The requestor must complete the Reservation form, which can be found after agreeing to the Use Policy at the bottom of this page.
- Cancellation Instructions
To cancel a reservation, notify the facilities coordinator. Include a description of the reserved item and the date of the reservation.
- Conditions and Limitations for Use
All arrangements will be in accordance with the following rules and conditions for use of the resources.
- Travel Log:There are two logs that are given to you along with the van keys. Those must be completed and signed when the van is returned.
- Gas: If the gas tank is less than 1/2 full when returning, you must fill the tank at the UNC gas station on Martin Luther King Blvd. using the gas card given to you in the pouch.
- Cleanliness: The vans must not have any trash or leftover items when returned.
- On Time: The parking space must be vacated by the time that was agreed to. The parking permit and vans must be returned by the time scheduled.
- Smoking: Smoking is prohibited in any of the vans.
- Responsibilities
You may be held responsible for any damage or loss related to the vans. We are not responsible for anything left in the vans.
