UNC Campus Health Documentation
If you are already a UNC Chapel Hill student and have already submitted your health documentation, you should check the link from Step Two and make you’re your documents are up to date. If they are, continue to the UNC Eshelman School of Pharmacy health documentation instructions.
Please carefully read all steps and the FAQs prior to taking any action so that you have a better understanding of the complete health documentation process.
Step One
Confirm that you have fulfilled all vaccinations required by UNC Chapel Hill Campus Health, and that you can provide documentation for these vaccinations. For required Immunizations and Medical History Instructions please click here.
Step Two
Complete the Immunization and Medical History form. The form can be found on the http://my.unc.edu home page, under the 'Enrolling Student' heading. You are required to sign in with your onyen and password. Once you complete the online form, print two copies of the PDF of Immunization Record generated by the site. One is for UNC-Chapel Hill Campus Health, and one is for your own records.
Step Three
Have your medical provider sign or stamp BOTH copies to verify your immunization records, or you can attach verified copies of your immunization record to the form. You may need to provide the health care provider with supporting documents if they are not already present.
Step Four
Send the verified immunization record to UNC-Chapel Hill Campus Health by May 15th, via certified mail with return receipt, or use a private carrier with signature required upon delivery. The address to send the form is located on the health immunization instructions. You can also find it here:

UNC Eshelman School of Pharmacy Health Documentation
Step One
Begin the Hepatitis B vaccination series by December 15, 2011, and complete it by June 15, 2012. This is mandatory in order for you to begin the PharmD program in the fall. Always keep your own copy of your vaccination record(s).
Step Two
Complete TB test
- Option 1: QuantiFERON (QFT) Gold - TB blood test (Recommended method). This is a one-time blood test and must be administered after March 31st. Please obtain lab results and submit with other documents.
- Option 2: Complete two TB skin tests. They must be administered one to three weeks apart and after March 31st. Have your results written on the Skin Test Result Form.
Keep copies of the lab/test results for your records.
Please Note: If the results for either test are positive, you will be required to have a Chest X-Ray taken. If the Chest X-Ray is NORMAL, you will need to meet with a designated provider in Campus Health Services here at UNC Chapel Hill, who will evaluate you for clearance. Once cleared, have this form completed by the provider. Please keep a copy for your records and bring a copy to the Office of Student Affairs. You will be required to have an evaluation once a year while you are in the program. If the Chest X-Ray is ABNORMAL, please see your physician or health department immediately.
Step Three
Make sure you have documentation of two Varicella (chickenpox) vaccine doses or the results of a varicella titer by May 15th. You should have a titer drawn if you have contracted varicella in the past or if you do not have documentation of receiving the vaccine. If your varicella titer comes back negative for immunity, you will need to get two doses of the vaccine. Keep a copy of your varicella document(s) for your records.
Step Four
Scan your health documentation (for the Hep B vaccination series, TB tests/evaluation, and varicella titer/vaccinations) into a pdf file with the title corresponding to “last name, first initial.pdf.” For example, Andrew Clapper’s pdf file would be called “Clapper, A.pdf.”
You can find machines to scan your documentation in many places. For example: school libraries, public libraries, drug stores with photo shops, wholesale stores, 24 hour office stores, and office supply stores.
Email your pdf file as an attachment to pharmacystudentaffairs@unc.edu by June 15th.
Keep your copy of your immunization records for yourself in a safe place.
Step Five
Have a Hepatitis B titer drawn by September 15th. Scan your titer results into a pdf file and send them via email to pharmacystudentaffairs@unc.edu by October 1st.
A quantitative hepatitis B titer is required for all students. If you received the Hep B vaccine in the past, you can have the titer drawn and turned in with your other documents.
Health Documentation Frequently Asked Questions
Do I need to send my Hep B vaccination series documentation if I have had a Hep B titer with positive immunity results?
Yes, our clerkship sites require that you have both.
Why do I have to send health documentation to two different locations?
UNC Campus Health mandates that you have certain health requirements completed in order to be a UNC-Chapel Hill student. The UNC Eshelman School of Pharmacy requires additional documentation because you will be in contact with patients in a health care setting when you are engaged in your practice experiences (clerkships). Our health requirements also protect you and your colleagues when you are conducting labs in the PharmD program.
Why doesn’t Campus Health send the UNC Eshelman School of Pharmacy my health record, or vice versa?
UNC Chapel Hill Campus Health does not send health documentation to individual schools, and the UNC Eshelman School of Pharmacy does not send health documentation on students’ behalf.
What will happen if I send health documentation to the UNC Eshelman School of Pharmacy that is not complete?
We will respond to your email and ask you to send all of your documentation in one pdf file once it is complete.
Do I need to send Campus Health some health documentation if I am already a UNC Chapel Hill student?
It depends on when you started school, and whether or not your immunizations have expired. Please check the document in step one of the Campus Health instructions for more details.
Do I need to have a Hep B titer done before July 15?
No, your Hep B titer is due October 1.
Do I still need a Hep B titer done if I have had all three Hep B vaccinations?
Yes, every PharmD student must have a Hep B titer done. The varicella requirement allows for either two vaccinations or a titer, but the Hep B requirement includes both the three vaccinations and a titer.
Can my Hep B vaccinations and titer be several years old?
Yes, they can be several years old. There is no time limit for the Hep B series and titer.
