Incoming ECSU-based students need to send health documentations to three locations: UNC-Chapel Hill Campus Health Services, the UNC Eshelman School of Pharmacy, and ECSU Student Health Services. You should send your health documentation to ECSU after you have sent your documentation to the other two locations.

Instructions for sending health documentation to ECSU

1. Print and complete the Report of Medical History Form.
2. By July 15th turn in the Report of Medical History Form and one copy of all of the immunization records you compiled for UNC Campus Health and the UNC Eshelman School of Pharmacy.
3. The documents can be delivered in person to 102 Cardwell/Hoffler Infirmary or sent via certified mail with return receipt to ECSU Student Health Services, 1704 Weeksville Rd, Elizabeth City, NC 27909.
