Incoming Asheville-based students need to send health documentations to three locations: UNC-Chapel Hill Campus Health Services, the UNC Eshelman School of Pharmacy, and UNC Asheville Health Services. You should send your health documentation to UNC Asheville after you have sent your documentation to the other two locations.

Instructions for sending health documentation to UNC Asheville

1. Print and complete the UNCA Immunization Record form.
2. The form may be delivered in person to Weizenblatt Hall, sent via certified mail with a return receipt to the address below, or faxed to 828-251-6887.
