MTM

Delivering Medication Therapy Management Services in the Community

Delivering Medication Therapy Management Services in the Community is an innovative and interactive practice-based activity that explores the pharmacist’s role in providing MTM services to patients. Pharmacists have a tremendous opportunity to receive reimbursement for monitoring and improving medication use in patients with complex medication regimens. The activity teaches pharmacists the essential skills necessary to become successful MTM practitioners. This certificate program will enhance pharmacists’ clinical expertise in evaluating complicated medication regimens, identifying medication-related problems, and making recommendations to patients, caregivers, and health care professionals.

The goals of the activity are to:

  • Advance public health and patient care through improved medication use,
  • Provide training to enhance pharmacists’ ability to effectively provide medication therapy management services,
  • Motivate increased numbers of pharmacists to establish MTM services, and
  • Communicate benchmark practices for providing MTM services.

Delivering Medication Therapy Management in the Community” is conducted in two parts

  • A self-study learning component consisting of four modules.
  • A live interactive training seminar consisting of case studies and hands-on patient interview and assessment practice sessions.

The first part — the self-study learning component — is designed to ensure that all participants have a solid understanding of the role of pharmacists as health care providers of MTM services. The self-study includes four learning modules that present in-depth information on the role MTM will play in the future of pharmacy practice, a plan for becoming a successful MTM practitioner, the essential skills and knowledge necessary for delivering MTM services, and how to conduct an MTM encounter. The self-study includes a self-assessment test and a pre-seminar exercise, that involves two patient interviews and is designed to help reinforce and evaluate participants’ understanding of key information and concepts.

Successful completion of the self-study learning component results in ten hours of continuing pharmacy education credit (1.0 CEU) – Universal Activity Number: 202-999-07-089-H04. The live training seminar is approved for eight hours (0.8 CEU) of continuing pharmacy education credit (UAN 202-999-07-236-L04). The post-seminar exercise is approved for 3 hours (0.3 CEU) of continuing pharmacy education credit (UAN 202-999-08-098-H04-P). Note: Final credit may be provided under different UANs than listed above. Statements of continuing pharmacy education credit and Certificate of Achievement will be mailed by APhA. The license partner program coordinator should indicate in the participant database whether statements of credit and certificates of achievement should be mailed to the license partner for distribution or directly to participants.
