Current applicants please note: the Phase II application will be available starting August 17, 2009.

After you have completed your Phase I  general application, you should complete the Phase II supplemental online application specifically for the UNC Eshelman School of Pharmacy. Please note that the requirements for different pharmacy schools (even those within the same state) may differ substantially.

Phase II of your application must be submitted online to the UNC Eshelman School of Pharmacy’s Office of Student Services by the postmarked date of November 2 for the following fall term. 

Before you begin the online supplemental application, it is important that you carefully read the following UNC Eshelman School of Pharmacy policies:


After
 submitting the Phase II online supplemental application, you must download the supplemental application signature/confirmation form and mail the completed form, along with your application fee, to the School. The signature/confirmation form will be available for download beginning August 17, 2009.

If you are claiming North Carolina residency, complete the online residency short form. After filling out the online residency form, you will need to print the confirmation page. Please note that if you have lived in North Carolina for less than three years, you must download and complete the residency long form instead of completing the online short form.

Mail the completed supplemental application signature/confirmation form, the North Carolina residency short form confirmation page or the North Carolina residency long form, and the $77 supplemental application fee to:

Please make checks payable to the UNC Eshelman School of Pharmacy.

It is important to note that these items must be sent together and postmarked by the November 2 deadline. Incomplete packets will be returned to the sender. We highly recommend that you send your packet using certified mail since we cannot confirm receipt of your Phase II application.
